Document Request Form
In order to process your start/stop service request, Aqua will need a copy of your official lease or settlement paperwork (proof of sale) with one state issued ID (please see list of options below).
When sending the lease/settlement, please make sure you have the signature page included. It will delay processing time if that is missing.
The form of identification must be one of the following government-issued photo IDs:
- Driver's licenses or other state issued ID by Department of Motor Vehicles
- U.S. Passport
- U.S. Passport Card
- U.S. Department of Defense ID
- Permanent Resident Card
- Border Crossing Card
- DHS-designated Enhanced Driver's License
- Federally recognized tribal-issued photo ID
- Employee ID
- HSPD-12 PIV Card
- Foreign Government-Issued Passport
- Canadian Provincial Driver's License
- Indian or North Affairs Canada Card
- Transportation Worker Identification Credential
- U.S. Citizenship & Immigration Services Employment Authorization Card (I-766)
- U.S. Merchant Mariner Credential
